Sage Urgent Care is at 369 Springfield Avenue in Berkeley Heights — about 2 miles from Summit High School and a short drive from anywhere in Summit. Take Springfield Avenue south through New Providence, or hop on Route 24 to Mountain Avenue. We see student athletes from Summit High School, Lawton C. Johnson Summit Middle School, Oratory Prep, Kent Place, and the Summit YMCA athletic programs every season.
New Jersey requires a pre-participation physical evaluation (PPE) every 365 days for student athletes. The form — the NJ Department of Education Annual Athletic Pre-Participation Physical Examination Form (often called the "blue form") — must be completed by a licensed physician, advanced practice nurse, or physician assistant. We complete this form for you the same visit.
What a sports physical at Sage includes:
- Health history review (you can fill out the parent/guardian portion in advance)
- Vital signs: blood pressure, heart rate, height, weight, BMI
- Vision screening
- Heart and lung exam (auscultation)
- Abdominal exam
- Musculoskeletal screening (joints, flexibility, strength)
- Discussion of any concerns — concussion history, recent injuries, supplements, mental health
- Sign-off on the NJ-approved athletic participation form
When to schedule (Summit sports timing)
- Fall sports (football, soccer, cross-country, field hockey, tennis): Get the physical by mid-August. Summit High School typically starts pre-season around August 15–20.
- Winter sports (basketball, wrestling, swimming, indoor track): Get the physical by early November.
- Spring sports (baseball, lacrosse, softball, golf, spring track): Get the physical by late February.
- Year-round/club athletes: Schedule for whenever your current physical expires — they're only valid for 365 days.
What happens at the visit
- Check in. Bring the partially-completed school physical form (the parent/guardian history section), insurance card if using insurance, and your athlete.
- Vital signs. Brief screening with our medical assistant.
- Provider exam. A physician completes the full exam and reviews the health history.
- Form sign-off. The form is signed and stamped before you leave.
- Follow-up only if needed. Most athletes are cleared on the spot. Rarely, a finding (heart murmur, blood pressure issue) requires further evaluation before clearance.

Bring documentation from the treating provider if available. Most controlled conditions don't affect athletic clearance. We document the condition on the form and work with the school's athletic trainer when needed.
