Our providers complete the full required exam and fill out whichever form your district uses — Universal Child Health Record, school district physical form, sports clearance form (combined with an annual physical), or out-of-state forms. How we handle it
- Vital signs. Height, weight, BMI calculation, blood pressure, pulse, vision and hearing screen.
- Full physical exam. Heart, lungs, abdomen, skin, ENT, lymph nodes, musculoskeletal, neurological screen, and growth/development assessment appropriate for age.
- Health history review. Bring a list of current medications, known allergies, prior surgeries, hospitalizations, and family medical history. We confirm immunization status during the visit.
- Form completion. We complete and sign whichever school or district form you bring. If you don’t have a form, we provide our own standard NJ-acceptable physical clearance documentation.
- Vaccinations if needed. If your child is missing required immunizations for school, we can administer them same-visit (flu shot, tetanus booster, TDAP). Some district-required vaccines may need to be coordinated with your pediatrician.
What happens at your visit
- Walk in any day, 8 AM to 8 PM. No appointment needed. Bring your child’s form, immunization record, and insurance card.
- Quick check-in. 5-minute intake at the front desk.
- Exam by a provider. Standard pediatric physical exam, age-appropriate. Typically 20–30 minutes.
- Form filled out before you leave. Signed and dated, ready to submit to the school office.
- Vaccinations if needed. Administered same visit when appropriate.
