New Jersey school districts require a current physical exam for kindergarten entry, periodic exams during elementary and middle school, and a physical for high school sports participation. Most pediatricians can do these — but appointments often book weeks out, especially during the August back-to-school rush. Sage Urgent Care offers walk-in school physicals 7 days a week with no scheduling lag.

Our providers complete the full required exam and fill out whichever form your district uses — Universal Child Health Record, school district physical form, sports clearance form (combined with an annual physical), or out-of-state forms. We’re experienced with the forms used by Berkeley Heights, Summit, New Providence, Chatham, and Murray Hill school districts, plus private and parochial schools throughout the region.

When to come in

Important: If your child has a complex medical condition, ongoing specialist care, or anything requiring detailed continuity of care, the school physical is best done by their regular pediatrician. We handle routine school physicals for healthy children — if anything in the exam suggests further evaluation is needed, we’ll provide clear referral guidance.

How we handle it

What happens at your visit

  1. Walk in any day, 8 AM to 8 PM. No appointment needed. Bring your child’s form, immunization record, and insurance card.
  2. Quick check-in. 5-minute intake at the front desk.
  3. Exam by a provider. Standard pediatric physical exam, age-appropriate. Typically 20–30 minutes.
  4. Form filled out before you leave. Signed and dated, ready to submit to the school office.
  5. Vaccinations if needed. Administered same visit when appropriate.
