Sage Urgent Care offers walk-in sports physicals, school physicals, DOT/CDL physicals, vaccines, and other preventive care. Most physicals complete in 30 minutes. Open 7 days, 8 AM to 8 PM.
Not everything that brings you to a clinic is an emergency. Sage Urgent Care handles the routine wellness needs that fit into the urgent-care model — sports and school physicals, DOT/CDL exams, flu shots and other vaccines, work clearance, and basic preventive care. All walk-in, no appointment, completed same visit.
Sports and school physicals are $50 self-pay with on-site form completion — we sign and stamp the NJ Department of Education athletic pre-participation form (the standard NJ "blue form") before you leave, accepted by all NJ public and private schools. DOT and CDL physicals are $90–$120 and most complete in under 45 minutes.

$50 self-pay. The price includes the visit, the exam, and the NJSIAA pre-participation form completion. No appointment needed.
A sports physical (pre-participation physical examination, PPE) focuses on cardiovascular and musculoskeletal evaluation specific to athletic participation. A school physical is broader, covering general health, growth, immunization status, and other school-required items. Sage provides both.
Usually 30–45 minutes total — including vision testing, hearing screening, blood pressure, urine drug screening if required, and the physical examination itself. The medical card is issued same visit if you pass.
Insurance coverage for sports physicals varies — most plans don't cover sports physicals as a preventive service, which is why we offer the $50 self-pay rate. DOT physicals are typically self-pay since they're for employment. Annual physicals through your PCP are different and are usually covered.
Yes. Annual flu shots are available throughout flu season for adults and children ages 4 and up. Most insurance covers flu vaccination at no out-of-pocket cost. Self-pay rate is reasonable.
Yes. The NJ Department of Education form we complete is accepted by every public and private school in New Jersey, including Union Catholic, Oak Knoll, Newark Academy, Pingry, and area private schools.
Photo ID, insurance card if using insurance, the school or sports form (we have NJ forms on hand), vaccination records, any current medication list, and the patient (especially for minors — a parent or guardian needs to be present for kids under 18).
